MICHAEL SHANE MCINTOSH, 46, of Pembroke Pines, Fla., died Sunday, November 17 at St. Vincent Hospital of Salem.
He was born October 21, 1967 in Salem to Ronald and Shirley Green McIntosh.
He is survived by his father:  Ronald McIntosh of Campbellsburg.  He was preceded in death by his mother: Shirley Green McIntosh.
There will be no visitation or services.

DONALD GENE SCHULER, 61, Charlestown, died Monday, November 25, 2013 at his home.
He was the founder and pastor of Harvest Worship Center in Charlestown, and a member of Graceland Baptist Church, owner of J & J Plywood, and previously employed by Whayne Supply Company.
He is survived by his loving wife, Barbara (Knipp) Schuler; his daughters; Angela Arndt (David), and Amiee Stout (Kenneth); his sons, Andrew Schuler, Aaron Schuler, and Donovan Clayton. His grandchildren: Kailen Grace, Kenneth Peyton, and Kendall Wayne; his sisters, Barbara Nalley, June Cooke, and Joanna Waggoner; brother –in-law, Arnett Knipp; sister-in-laws, Carolyn Boling (Bo), Doris Funk (Charlie), Edna Swiech (Gene), Shriley Scoggins (Gene), Wanda Schroeder (Jack), and Delores Knipp, and many other family members. He was preceded in death by his parents, Floyd and Anna Mae (Graft) Schuler; brother, Carl Joseph Schuler; grandson, Joshua Andrew Schuler.
Funeral services will be held on Saturday, November 30, 2013 at 1pm at the Graceland Baptist Church, with burial to follow in the Abundant Life Memorial Gardens in New Albany. Visitation will begin on Friday from 12pm to 8pm at the Grayson Funeral Home in Charlestown, and Saturday from 9am till 11:30am at the funeral home.

FREDDIE HAYES, 65, of Scottsburg, died Monday, November 25, 2013 at the Hosparus Inpatient Care Center in Louisville.
He was born on September 9, 1948 in Scottsburg to Kelly and Marie Bowling Hayes. He was a maintenance engineer for ILPEA Industries in Scottsburg, a U.S. Army Veteran of the Vietnam War, a life member of the Scott County V.F.W. Post # 6582 and ABATE.
Survivors include his wife, Terry L. (Alford) Hayes; two sons, Jeff Hayes of Seattle, Washington and Jacob Hayes of Scottsburg; his mother of Silver Springs, Fla.; two brothers, Quinn and Bobby Hayes both of Silver Springs, Florida; a half-brother, Kelly Glen Hayes of Scottsburg, and one granddaughter, Brianna Lynn Hayes.
He was preceded in death by his father.
Funeral Service: 1:00 pm Saturday, November 30, 2013 at Collins Funeral Home with with burial in Little York Cemetery.
Visitation: 10 am to 1 pm on Saturday.

BILLY WAYNE AKEMON, 67, of Charlestown, died Tuesday, November 26, 2013 at Jewish Hospital in Louisville.
He was born on May 7, 1946 to Jim and Evelyn (Grills) Akemon in Harlan Co., Ky. He was a retired service tech for Sears Company, a laborer in the construction industry, a member of the Local 795 Labors and Hod Carriers, and a member of the Faith World Church in Charlestown.
Survivors include his wife, Darlene (Braswell) Akemon; sons, Scott Akemon (Jennifer) of Rome, Ga., Jack Aaron (Teresa) of Marysville, IN, Jeff Aaron (Susan) of Charlestown, and Brian Aaron (Jamie) of Charlestown; brothers, James E. Akemon (Sharon) of Charlestown, and Mike Akemon (Darlene) of Charlestown; sisters, Doris Bower (Ronnie) of Charlestown, and Linda Carlisle (Calvin) of Charlestown; grandchildren, Catie, Lauren, Josh, John, Jon,  Hannah, Donnie, Connor, and Caleb.
Funeral services will be held on Monday, December 2, 2013 at 11am at the Grayson Funeral Home, with burial to follow in the Charlestown Cemetery.
Visitation will begin on Sunday, December 1, from 11am to 7pm, and after 9am on Monday.

TJ (POP) HART, 82, of Salem, died Tuesday, November 26 at Floyd Memorial Hospital.
He was born January 26, 1931 in Woodbine, Ky. to William and Lucy Walters Hart.  He was a retired employee of the City of Salem Street Department. He was an Army veteran.
He is survived by four sons:  Stacy Hart (Kristie) of Campbellsburg, William J. Hart (Milly), Jimmy and Tommy Hart all of Salem, a daughter:  Ruth Cole (Tim) of Campbellsburg, a brother:  George Hart of Corbin, Ky., 10 grandchildren and 19 great-grandchildren. He was preceded in death by his wife:  Maggie Hart, a son:  Jeff Hart, three brothers:  Charlie, Paul, and Leland Hart, two sisters:  Imogene Roop and Lelah Roop and 3 great-grandchildren.
Funeral services will be held Saturday, November 30, 2013 at 1 pm at Weathers Funeral Home.  Burial will follow in Crown Hill Cemetery.
Visitation will be held Friday, November 29, from 4 – 8 pm and Saturday from 9 am until the time of services.

TARA LEA TALBOT, 47, of Palmyra, died Sunday, November 24, 2013.
She was born on August 16, 1967 in Alexandria, Va. to the late Robert L. Talbot and Ernestine Griffin, who survives.
Other survivors include her sons, Jere'ad (Tonya) Hausberger Sr. of Palmyra, and Kristin Hausberger of Kentucky; her daughter, Raven Hausberger of Kentucky; her brother David (Blanch) Talbot of Michigan; her sister, Elizabeth Sue (Dennis) Lawrence of North Carolina; her step-brother, Tim Griffin; her step-sister, Tara Abbott; and her grandchildren, Ariah, Isabella and Jere'ad Hausberger Jr., Sierrah Bostock and Damian Bedford.
She was preceded in death by her brother, Robert L. Talbot Jr.
There will be no funeral service.

PAUL J. MIDDENDORF, 66, of Salem, died Tuesday, November 26 at Floyd Memorial Hospital.
He was born July 10, 1947 in Louisville to Frank and Doris McGown MIddendorf.  He was a retired teacher for Mitchell Community Schools and a retired employee of Hoosier Uplands in Mitchell. He was also a member of Salem Moose Lodge.
He is survived by his life partner: Thomas G. Ernst of Salem and a brother: Robert Middendorf of Olathe, Kan. He was preceded in death by his parents.
Cremation was requested with no visitation or services.

BRENDA KAYE CHIZEK, 50, of Jeffersonville, died Friday November 22, 2013 at her residence.
She was born on September 11, 1963 in Barstow, Calif. to Richard and Sandra Satterthwaite Chizek.
Survivors include her parents; her daughters: Lindsey Chizek of Jeffersonville and Nichole Shortlidge of Virginia; and her sister: Cynthia Marie Booker of Barstow.
There will be no services held at this time. Cremation was chosen.

BARBARA ROSE WEBB, 78, of Campbellsburg, died Sunday, December 1, 2013 at her home.
She was born August 9, 1935 in Riverdale, Ind. to Joseph E. Schultz and Nina Jones Schultz Staggs.  She was a former employee of B.F. Goodrich and a retired employee of Panasote. She was a member of Saltillo Christian Church.
She is survived by her sons:  James F. Webb Jr. (Debbie) of Orleans, Jeffrey A. Webb (Nina) of Campbellsburg and Richard D. Webb (Cindi L.) of Salem, three sisters, four half-brothers and two half-sisters; 14 grandchildren and 17 great-grandchildren. She was preceded in death by her husband: James Webb Sr., a son: Larry Eugene Webb, two brothers and her parents. 
Funeral services will be held at 6 pm on Wednesday, December 4, 2013 at Weathers Funeral Home. Cremation will follow. Burial will be held at a later date in Silverville Baptist Cemetery.
Visitation will be from 4 to 6 pm on Wednesday.

BLAKE L. BURNS, 95, Scottsburg, died Tuesday, November 26, 2013 at his home.
He was born on June 2, 1918 in Salem to Reverend Harvey S. and Clara Batt Burns. He was the former and first Mayor of Scottsburg, a U.S. Army veteran of World War II, and a member of the First Baptist Church of Scottsburg.
Survivors include his wife, Sue Burns; his daughters, Marsha Reeves-Fewell of Scottsburg and Christina (Christy) Beardsley of Zionsville, Ind.; his grandchildren, Scott Reeves, Kristin Reeves Pond (Charlie), Brian Burns Beardsley and Jacqueline Robison (Ben); and several nieces and nephews. He was preceded in death by his sister, Virginia Burns Ball; and his sons-in-law, Dave Reeves and Terry Fewell.
Funeral services were held Monday, December 2, 2013 at Scottsburg First Baptist Church with burial in the Scottsburg Cemetery.

ROBERT R. CAMPBELL, 54, Pekin, died Monday, November 25, 2013 at his residence.
He was born on May 4, 1959 in Salem to the late Raymond Robert and Emma Jean Sparks Campbell, who survives. He was an employee of Kimball Office Furniture in Salem and a former employee of the old Ashland Service Station in Borden. He was also member of the Tabernacle of God in Pekin.
Survivors include his wife: Patricia Ann Gibson Campbell; his daughters: Amanda Joann McGluathlin of Fredericksburg and Alaina Dawn McLemore of Salem; his sister: Roseanne Callahan of Jeffersonville; five grandchildren and four step-grandchildren.
Funeral services were held Sunday, December 1, 2013 at the Tabernacle of God. Burial followed at Mt. Washington Cemetery in Pekin.
Dawalt Funeral Home handled the arrangements.
